Flooring lag screws are failing

Two lag screws attaching the plywood floor to the subfloor have come loose. The screws are in the South desk. One of the plywood sheets has risen about 1/4" above the rest of the floor, resulting in a tripping hazard in the dark. No one has volunteered to work on this task yet.
